Emergency Medical Authorization

In the event of illness or injury while under the care of Regal Rovers Dog Services, every effort will be made to contact the owner. If contact cannot be made, I authorize Regal Rovers to seek veterinary care and approve emergency treatments up to the amount entered below per pet. I assume full financial responsibility for all veterinary charges incurred.

North Carolina Rabies & Health Compliance

I verify that my dog is current on all core vaccinations. In strict compliance with N.C.G.S. § 130A-185, I confirm my dog is vaccinated against rabies. I agree that my dog will wear a current rabies tag on their collar at all times during Regal Rovers services. I understand that Regal Rovers reserves the right to refuse service if my dog shows signs of infectious illness.

Regal Rovers agrees to keep all keys and access codes strictly confidential. Physical keys will not be labeled with the client's name or address.

Cancellation & Liability Waiver

Cancellations: Services canceled with less than 24 hours' notice will be billed at 50% of the scheduled service rate.

Liability: I understand that working with animals carries inherent risks. I agree to hold Regal Rovers, its owner, and The Alexander Collective harmless from any claims, damages, or injuries resulting from my dog's behavior, including property damage or injury to other animals, except in cases of gross negligence.
